DISTINGUISHING FEATURES OF THE CLASS

Under general supervision, an employee in this class is responsible for the evaluation and design of forms used in the data processing and manual administrative procedures.  The incumbent analyzes the information needs of a department and develops new or revised forms to process and maintain information more effectively.  The incumbent exercises a considerable amount of independent judgment and initiative in the performance of the required duties.  Supervision is received from technical and administrative supervisors through conferences and review of forms developed.  Does related work as required.

 

TYPICAL WORK ACTIVITIES

         Designs forms to be used in record keeping and reporting by means of data processing and manual administrative procedures;

         Evaluates existing forms and makes recommendations on the format and content to include necessary information and to reduce duplication, inventories and preparation time;

         Confers with user personnel in the design of printed forms;

         Prepares and issues written instructions for the use of forms;

         Examines, corrects and approves the content and format of vendor proofs for adherence to purchase specifications for printing and binding;

         Maintains an inventory control and master file of all forms printed and distributed;

         May calculate the cost savings resulting from the development of new or revised forms;

         Maintains contact with manufacturers to keep informed of new form innovations.

 

FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS

Thorough knowledge of the principles, practices and procedures of forms analysis and design; good knowledge of the reporting and data collection requirements relevant to the administration of a governmental department; ability to learn the principles, practices and procedures of data processing information input and retrieval systems; ability to maintain systematic records and files of forms being used and available from vendors; ability to express oneself clearly and concisely, both orally and in writing; ability to analyze and solve problems related to information management and retrieval; physical condition commensurate with the demands of the position.

 

MINIMUM QUALIFICATIONS

         OPEN COMPETITIVE

         Either:

         a)  Graduation from a New York State or regionally accredited college or university with a Bachelor's Degree, and three (3) years of experience in forms analysis and design; or,         

        

         b)  Graduation from a New York State or regionally accredited college or university with an Associates Degree, and five (5) years of experience in forms analysis and design; or,

        

         c)  Graduation from a standard senior high school or possession of a high school equivalency diploma, and seven (7) years of experience in forms analysis and design; or,

        

         d)  An equivalent combination of education and experience as defined by the limits of a), b) and c).

 

         PROMOTIONAL

         Two (2) years of permanent competitive status as a Forms Technician.
