FEATURES OF THE CLASS
Under general
supervision, an employee in this class is responsible for the training,
development and supervision of a staff of Home Health Aides who provide care to
chronically ill patients in their homes. The incumbent schedules and assigns
the work of the Home Health Aides, performs assessments of individual patients
in home care setting, develops patient care treatment plans, and demonstrates rehabilitative
nursing procedures to aides. Supervision
is received from an administrative supervisor through conferences and written
reports. Does related work as required.
TYPICAL WORK
ACTIVITIES
Orients, teaches nursing and homemaking procedures and supervises staff of Home Health Aides in the Department of Health Services;
Plans and
schedules Home Health Aide services to patients in an area of Suffolk County;
Develops a Home
Health Aide plan for each patient and teaches and demonstrates each procedure
to Home Health Aide;
Fosters patient
and family independence by teaching nursing techniques and activities of daily
living to families of patients receiving home health services;
Supervises the
maintenance of patient records and reports on all patients visited by Home
Health Aides;
Obtains necessary
financial data and information required for billing;
Reviews all Home
Health Aide placements evaluating whether appropriate care is being received;
Carries out level
of care evaluations for skilled nursing facilities and health related
facilities placement;
Monitors quality
of care and need for care provided by aides of other agencies serving Medicaid
patients.
FULL PERFORMANCE
KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS
Thorough knowledge
of the principles and practices of rehabilitative nursing specific to the needs
of the elderly and chronically ill in the home; knowledge of the total goals
and objectives in the Long Term Care Program; knowledge of current developments
and sources of information in the field of long term care nursing; ability to
safely draw intravenous blood; ability to establish and maintain effective
working relationships with other governmental facilities, community groups and
private agencies; ability to plan and supervise the work of the Home Health
Aides in a manner conducive to full performance and high morale; ability to
express oneself clearly and concisely, both orally and in writing; ability to
maintain accurate records and prepare comprehensive reports; physical condition
commensurate with the demands of the position.
MINIMUM
QUALIFICATIONS
OPEN COMPETITIVE
Graduation from a recognized school
of professional nursing and four (4) years of nursing experience, two (2) of
which must be in the field of Public Health community nursing.
At the time of
appointment and during employment in this title, the candidate must be
registered as a Professional Registered Nurse by the New York State Education
Department.
At the time of
appointment and during employment in this title, candidates must possess a
valid license to operate a motor vehicle in New York State.
