DISTINGUISHING FEATURES OF THE
CLASS
In accordance with the
requirements for the New York State Public Health Corps Fellowship Program,
Fellows will perform work related to the core mission of Suffolk County’s
Department of Health Services. Individuals will be assigned to a division
within the Department of Health Services to support current operations related
to COVID-19 response and recovery, and to increase preparedness for future
public health emergencies. Types
of assignments will be premised on level of education, knowledge, skills, and experience,
in conjunction with the Department’s needs. NYSPHC Fellows will concurrently be
enrolled in Cornell University’s Public Health Essentials Certificate Program
and other required professional learning opportunities. Work is performed under
general supervision. Note: appointments
to these titles are temporary in nature for a period of one year.
TYPICAL WORK ACTIVITIES
When assigned to the Division
of Patient Care Services the
Fellow will support the Division’s mission to assist vulnerable residents in
accessing needed health care services and providing education, case management,
and referrals. Tasks may include:
Assists with patient case management
activities;
Assists in the collection and
analysis of data relative to the performance of programs and operations as well
as data reporting to New York State and grant administrators;
Assists with conducting educational
programs for health care professional and community stakeholders;
Interfaces with the public; responds
to public health questions;
Assists with the operation of
vaccination clinics providing routine childhood immunizations as well as
vaccination against COVID-19 and other illnesses impacting the community.
Assists in making daily reports,
quality improvement reports, and keeping adequate inventory records of
medication and medical supplies for Jail Medical Unit and/or Immunization
Action Program.
When assigned to the Office of
Minority Health the Fellow will support the Office’s
core mission of improving health outcomes and eliminating health disparities in
minority populations by coordinating public health initiatives and implementing
educational programs that are culturally and linguistically appropriate, in
collaboration with community and faith based organizations. Tasks may include:
Assists in the collection and analysis
of data on economic, demographic, and sociological characteristics that have an
impact on the development of health policies and programs;
Conducts outreach and develops
relationships with hard to reach minority communities;
Assists in drafting and developing programs aimed at addressing
issues surrounding chronic conditions that cause increased risk of adverse
outcomes for certain communities.
When assigned to Division of
Community Mental Hygiene Services the Fellow will support the coordination and oversight of all community
services to persons with alcohol and substance use disorders, mental illness,
and developmental disabilities. Tasks may include:
Assists with coordinating public
forums and community meetings regarding available assistance programs;
Reviews and analyzes local
substance use disorder data;
Assists with drafting and
publishing reports;
Drafts and prepares public
messaging on available programs;
Assists with gathering data and
materials for resource directories for mental health services and substance use
disorder services.
When assigned to the Division
of Public Health the Fellow will support the
Division’s mission of preventing the spread of communicable diseases and
providing education, case management and referrals. Tasks may include:
Supports community vaccination efforts
against communicable diseases, including COVID-19, which may include Point of Distribution
(POD) organization/planning and/or on-site assistance with supplies and client
registration;
Assists the Department’s Central
Pharmacy with medication/vaccine distribution and other programs;
Assists with health promotion
activities by updating content for the Department’s website and social media
accounts, as well as engaging in social marketing;
Assists with case investigations
and surveillance activities for communicable diseases, which may include
patient interview, contact elicitation and notification, identifying patterns
and clusters of disease, and tracing chains of infection to their origin;
Assists with case management
activities;
Assists in the collection, analysis
and reporting of data.
Coordinates environmental
investigations, tracks cases, provides guidance on and tracks lead abatement
projects to ensure that lead hazards are remediated for the Childhood Lead
Poisoning Prevention Program;
Assists with field investigations
and laboratory work;
Performs data management, analyses,
reporting, and research;
Develops health educational
materials; coordinates and conducts community presentations;
Assists staff with contract
management, claims, and reimbursement for grants related to COVID-19 and other
public health initiatives;
Develops and leads epidemiological
investigations into COVID-19.
When assigned to the Division
of Environmental Quality the
Fellow will assist the Division in conducting comprehensive programs that
protect Suffolk County residents against adverse environmental factors
including groundwater and drinking water protection, wastewater management, toxic
and hazardous materials pollution control, monitoring and laboratory analyses,
enforcement of regulations, environmental
management studies, and
programs for ground and surface waters. Tasks may include:
Collects and logs samples, tabulates
data, and provides analyses;
Performs
educational outreach and communication and assists with implementing various
environmental programs, including Suffolk County’s
Reclaim Our Water Initiative;
When assigned to the Division of Preventive
Medicine the Fellow will assist the Division with education for residents
and the provider community, enabling them to make well-informed decisions about
lifestyle habits involving diet, exercise, smoking, etc. which affect their
overall health and wellbeing.
Tasks may include:
Performs educational outreach to
schools, businesses, and community organizations;
Collaborates with other agencies to
design various public health programs to provide education in an effort to
prevent disease, disability, and premature death;
Assists in conducting public health
education programs in schools, businesses, and community organizations concerning
topics such as tobacco cessation and enforcement, diabetes prevention,
nutrition, and promotion of physical activity.
When assigned to the Division
of Emergency Medical Services the Fellow will support
the Division’s core mission and responsibility of oversight and administration
of Suffolk County’s Emergency Medical Services system, and to provide support,
education and cooperation necessary to enable the emergency medical services
agencies to provide the best emergency medical care possible. Tasks may
include:
Assists with community outreach to
schedule and conduct Narcan/Naloxone trainings;
Drafts and edits educational
materials;
Collects data and conducts monthly
reporting;
Assists with analyzing and
implementing quality improvement initiatives for local EMS.
All Fellows will provide
COVID-19 response assistance as needed, which may include:
Conducts case investigation and/or
contract tracing related to COVID-19;
Participates in community support
related to COVID-19, which may include managing daily deliveries of
isolation/quarantine letters, food, and supplies to individuals with active
cases;
Responds to public inquiries about
current public health mandates related to COVID-19;
Conducts on-site registration at
vaccine PODs and assists in vaccine supply organization.
FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND

Good knowledge of public health principles,
practices and administration; working knowledge of public health methods of
disease detection, prevention and control; working knowledge of the principles and methods of public
speaking; working knowledge of Federal, State, and local Laws, codes, and
policies concerning social and health problems; working knowledge of public
health programs and services; working knowledge of trends, developments, and
literature in the field of public and environmental health; skill in the use of
standard business computer applications
and databases good knowledge of research methods; ability to gather, organize,
and evaluate data and make appropriate inferences; ability to obtain and verify
information through investigation and interview; ability to design and
implement studies for public health issues; ability to prepare accurate and
concise reports; ability to establish and maintain effective working
relationships with co-workers, stakeholders, and general public; ability to

MINIMUM QUALIFICATIONS

(a) Completion of a
minimum of sixty (60) credits from a college with federally-authorized
accreditation or registration by NY State, including or supplemented by
eighteen (18) credit hours in business administration, finance (including
accounting), public health, medicine, dentistry, nursing or pharmacology
programs, allied health, social services, communications, logistics planning,
education, public policy/administration, emergency health programs technology,
emergency health programs, or any other health-related degree; or,
(b) Graduation from
high school or possession of a high school equivalency diploma and two (2)
years of experience in a health or human services field, or in a position
involving significant public contact and/or administrative, technical, clerical
or other operational activities.
Public Health Fellow II
Graduation or proof
of anticipated graduation within one (1) semester of appointment, from a
college with federally-authorized accreditation or registration by NY State
with a Master's Degree in business administration, finance (including
accounting), public health, medicine, dentistry, nursing or pharmacology
programs, allied health, social services, communications, logistics planning,
education, public policy/administration, emergency health programs technology,
emergency health programs, or any other health-related degree.
Public Health Fellow III
Graduation, or proof
of anticipated graduation within one (1) semester of appointment, from a
college with federally-authorized accreditation or registration by NY State
with a Doctorate in business administration, finance (including accounting),
public health, medicine, dentistry, nursing and pharmacology programs, allied
health, social services, communications, logistics planning, education, public
policy/administration, emergency health programs technology, emergency health
programs, or any other health-related degree.
NECESSARY SPECIAL REQUIREMENTS
All levels of Public Health Fellows will be enrolled in Cornell
University’s Public Health Essentials Certificate Program, as well as other
professional learning opportunities.
All levels of Public Health Fellows will be expected to participate in a
NYSPHC Local Public Health Partnership Team.
