DISTINGUISHING FEATURES OF THE CLASS

Under general supervision, an employee in this class provides intensive case management services for clients in a mental health setting. The work is concerned primarily with planning and problem-solving to overcome obstacles faced by clients such as systems rigidity, fragmented services, under-utilization of services, and lack of accessibility to certain services and resources. Supervision is received from a professional superior through review of case records and reports, and individual and staff conferences. Does related work as required.

 

TYPICAL WORK ACTIVITIES

            Performs outreach services to engage clients;

            Monitors and coordinates the delivery of necessary evaluations and assessments in order that each client's needs are identified;

            Coordinates and participates in the development of an individualized goal-oriented service plan;

            Provides coordination and assistance in crisis intervention and stabilization;

            Assists the individual in achieving objectives and maximizing independence and productivity through on-site support, training and assistance in use of personal and community resources;

            Assists in developing formal and informal community supports and networks of relatives, friends and others;

            Advocates for changes in the system.

 

FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS

Thorough knowledge of community resources and how to use them effectively; ability to develop and maintain cooperative relationships with patients, colleagues and patients' families; ability to develop and maintain case work records; ability to express oneself clearly and concisely, both orally and in writing; ability to analyze information and reach sound conclusions; patience; good judgement; physical condition commensurate with the demands of the position.

 

MINIMUM QUALIFICATIONS

            OPEN COMPETITIVE

            Graduation from a college with federally-authorized accreditation or registration by NY State with a Bachelor's Degree in a human services field* and four (4) years of experience in providing services to mentally disabled** patients/clients.

 

NOTE:  A Master's Degree in a human services field* may be substituted for two (2) years of the required experience.

           

*For purposes of qualifying for this title a "human services field" includes Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Physical Therapy, Recreation or Recreation Therapy, Counseling, Community Mental Health, Child and Family Studies, Speech and Hearing, and Sociology.

           

**The term "mentally disabled" refers to persons diagnosed as suffering from mental illness, mental retardation, alcoholism or substance abuse.

 

NECESSARY SPECIAL REQUIREMENT

At the time of appointment and during employment in this title, employees will be required to possess a valid license to operate a motor vehicle in New York State.
