An employee in this class
develops and implements an adult day health care program at the John J. Foley Skilled Nursing Facility designed to
increase the sensual skills of the elderly and younger adults who have physical
or psychological conditions that require daily supervision, but do not require
an inpatient setting. These conditions include Alzheimer’s disease, stroke, Multiple
Sclerosis, head trauma, Parkinson’s disease, and diabetes. Duties include responsibility for developing
and monitoring therapeutic recreational activities and counseling programs. Supervision is exercised over therapists,
Registered Nurses, Nurses’ Aides, and recreation staff engaged in program
activities. Work is performed under
administrative and clinical supervision through conferences, submission of
reports, assessment of program development and achievement of objectives. Performs related work as required.
TYPICAL WORK ACTIVITIES
Designs
and implements a program capable of increasing the social skills of physically
handicapped, Alzheimer and infirm clients;
Assists
clients and/or their family members by advising them or referring them to sources
of assistance with problems they encounter in housing, health care, nutrition
or legal matters;
Maintains
statistics of program activities and client participation;
Supervises
therapists engaged in promoting the recreational and physical activities of
program participants;
Ensures
compliance with local, State and federal regulations related to Adult Day Care
programs;
Assures
that activities and services are provided in accordance with participants'
needs;
Trains
personnel in the handling of physically handicapped clients;
Prepares
reports on program activities and recommends changes.
FULL PERFORMANCE KNOWLEDGES,
SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS
Thorough knowledge of activities
and programs appropriate for senior citizens; good knowledge of geriatric
psychology and the physical capacities of senior citizens and the infirm; good
knowledge of applicable state and local laws related to senior citizen
programs; ability to plan and establish programs which are effective and
helpful to the clients; ability to establish and maintain good working
relationships with the State, clients and staff members; ability to keep
accurate records and write periodic reports; ability to express oneself clearly
and concisely, both orally and in writing; physical condition commensurate with
the demands of the position.
MINIMUM QUALIFICATIONS

COMPETITIVE
EITHER:
(a)
Graduation from a New York State or Regionally accredited college or university
with a Master’s Degree in gerontology, nursing, health or a related field; and
two (2) years experience in directing the delivery of health or rehabilitative
services to the frail elderly population, or;
(b) Graduation
from a New York State or Regionally accredited college or university with a
Bachelor’s Degree in gerontology, nursing, health or a related field; and four
(4) years experience in directing the delivery of health or rehabilitative
services, at least two (2) years of which must have involved working directly
with a frail elderly population.
